Release of Liability Consent

In consideration of privileges and participation in Dance On! 2025, the undersigned applicant does hereby waive, release, and discharge Arts in Motion Studio West Michigan, its directors, officers, members, and volunteers (the “released parties”) from any and all liability for death, injury, or other loss of any kind suffered by the applicant which arises out of participation in Dance On! 2025, regardless of whether the death, injury, or other loss was caused, in whole or in part, by the negligence of the released parties. By signing this release, the applicant acknowledges the inherent risk involved in the event’s physical activities and agrees to assume all risk of participating in Dance On! 2025. The applicant further agrees to indemnify and hold harmless the released parties from any and all liability incurred to third parties which results from death, injury, or other loss suffered by the applicant. All participants must read this release and sign below. A parent/guardian must sign for any participant under the age of 18.
